There's a bit of a learning curve but it's pretty flexible, and you have the options of downloading free assets OR buying OR creating your own assests.
It's a 2D thing, your map looks like the old pokemon or zelda games.
I know also some people like to use minecraft in sandbox mode, alternatively luanti (formerly minetest) is a free open-source alternative (also with a sandbox mode).
We have found Tiled Map Editor (free & open source!) to be a great tool for headspace / innerworld.
